What is forehead reduction surgery?

Forehead reduction surgery, also referred to as hairline advancement surgery, hairline lowering surgery or foreheadplasty, reduces the size of an elongated forehead by removing a portion of the forehead skin and creating a lower hairline or by doing hair transplant. The distance between the hairline and the eyebrows is effectively reduced. The extent to which the hairline may be advanced depends on variables such as hairline height, scalp laxity and the patient’s preferences.

Larger foreheads may be due to genetics, hair loss, or other cosmetic procedures. These options can help balance the proportions of your face and it’s different from a brow lift procedure.

Typically, the female hairline should begin between 4 and 6cm above the eyebrows, at the transition point between the vertical forehead and where the forehead slopes backwards towards the scalp. Forehead reduction surgery can help patients by bringing the hairline to a more pleasing position, thereby restoring facial harmony.

Types of forehead reduction procedure in Iran

1.Surgical forehead Reduction: A surgical approach which moves the scalp forward and removes a portion of the forehead skin.

2. Non-Surgical Forehead Reduction: non-surgical Forehead reduction techniques will rejuvenate the upper part of the face.

There are a number of non-surgical forehead reduction techniques available in Iran which are as follows:

. Hair transplant: A procedure which makes the forehead appear smaller by grafting hair to the forehead making the hairline appear smaller.

Hair transplant is a non-surgical procedure used in a forehead reduction procedure which is less invasive than the surgical procedure but more invasive than the following procedures.

. Botox Injections: Botox injection in addition to being used to remove wrinkles, it can also change the location of the eyebrows if injected above the face.  By carefully injecting Botox into the muscles that lower the eyebrows, you can release the eyebrows to fit in the correct place.

. Gel injection: face lift with gel Injection is a non-invasive method, safe and there is no need for recovery time. For wrinkled skin, the gel is injected into the wrinkles and, because of its filling properties, removes wrinkling of the skin.

. Hyperthermia and radiofrequency (RF): is all about stimulating your own collagen production. During a treatment a machine is used to heat the skin to between 38-40 degrees with radiofrequency, which feels like a hot stone face massage – you do feel heat in the skin but it’s comfortable.

These 4 methods are the least invasive methods used to lower and also rejuvenate forehead skin.

Before Forehead reduction Surgery

Preparing for Forehead Reduction Surgery

Being in the best possible state of physical and mental health prior to surgery is one of the most effective ways to ensure the best possible outcome.

Here are some tips to help you prepare for surgery:

. Disclose your complete medical history to your surgeon. It is particularly important you let your surgeon know if you suffer from an eye-related condition like glaucoma, dry eye or detached retina. Other medical conditions such as high blood pressure, a history of keloid scars or circulatory disorders should also be shared with your surgeon.

. Inform your surgeon of any previous plastic surgery you have had performed: for example, if you have had a coronal brow lift this will render you unsuitable for forehead reduction surgery.

Let your surgeon know all of the medications you are taking. You will be instructed to cease taking aspirin and anti-inflammatory medication such as Advil several weeks prior to surgery to minimize bleeding and bruising.

Your surgeon will also provide you with a list of over-the-counter medications or health supplements to avoid. Some natural medications and vitamins may exacerbate bleeding.

Avoid alcohol and salty foods for the week leading up to surgery to decrease fluid retention and swelling.

Stop smoking at least one month prior to surgery. Smoking can have a detrimental impact on your healing from surgery and in some cases, even cause tissue death (necrosis).

Get into an optimal state of health before surgery: eat nourishing and wholesome food, rest, exercise, drink plenty of water. A healthy mind and body will help guarantee a quick recovery and expedite healing.

By carefully adhering to your surgeon’s preoperative guidelines before your forehead reduction procedure, you can do your part to guarantee the best possible results from surgery.

Foreheadplasty benefits

. Shortens the distance between the eyebrows and hairline, restoring proportion and balance to the face.

. Can be performed in conjunction with a brow lift, which smooths most forehead wrinkles.

. Overall improved appearance

. A more youthful appearance

. Reduction of temporal hair loss

. The alleviation of wrinkles in the upper forehead

. Alteration of the hairline (for example, surgery can eliminate or create a widow’s peak).

Am I a Candidate for Hairline Lowering?

Individuals who are unhappy with the size of their forehead are ideal candidates for this procedure. Generally, women are likely to enjoy better results as they do not suffer from hairline recession to the same extent as men.

However, men can still benefit from surgery but first require a thorough assessment to determine their suitability. Men who undergo forehead reduction surgery need to be cognizant of the progressive nature of hair loss in males, and realize that further procedures such as hair transplants may be necessary to maintain optimal results.

For men who are undergoing facial feminization surgery (FFS), the forehead reduction procedure is key to achieving a more feminized facial appearance.

The best candidates for forehead reduction surgery will fit some of the following criteria:

. A high hairline and a large forehead

. Individuals with low or heavy brows, as in some cases forehead reduction surgery can help elevate the brows to a more attractive position.

. Individuals with supple forehead skin and scalp that has a certain degree of laxity.

. Individuals in a sound state of mental and physical health

. Individuals with realistic expectations about what surgery can achieve

. Men with thick hair density and no family history of hair loss

. Men who have a new hairline created by hair grafting

Risks & Complications

Similar to any surgical procedure, forehead reduction surgery carries risks. Honest communication with your surgeon about your medical history or any factors that may complicate your surgery or recovery can help to mitigate risks and maximize your results.

Be sure to also follow your surgeon’s advice as closely as possible before and after surgery: he or she will provide guidelines that will help you to safely prepare for the procedure and recover quickly afterwards.

However, some of the complications that can arise as a result of foreheadplasty include:

. Infection as a result of poor wound healing

. Bleeding

. Numbness at the incision site and in the front of the scalp until the nerves regrow; a small number of patients could possibly experience permanent numbness.

. Fluid accumulation at the wound site (seroma)

. Facial asymmetry (this usually corrects itself as healing progresses)

. Visible scarring (in the rare instances that this does occur, hair grafts can conceal scars).

. Individuals with darker skin tones may be left with a lighter, white scar that’s not so easily concealed.

. Bruising which may extend down to the eyes (bruising usually subsides within a couple of days).

. Hair loss around the scar: this may be temporary or permanent.

. Loss of mobility and expression in the forehead area (the effects are similar to Botox). This loss of mobility should be temporary, but in some cases can be permanent.

. Revision or secondary surgery may be necessary.

During Forehead Reduction Surgery

Forehead reduction surgery step by step

Forehead reduction surgery generally takes 2-3 hours and proceeds according to the following steps:

  1. The patient is sedated with local or general anesthesia, provided orally or intravenously.
  2. The surgeon marks the postoperative hairline and makes an incision. The incision is undulating or may follow a zigzag pattern to mimic the natural hairline.
  3. The skin above the marked line is excised to remove the excess forehead and the hairline is advanced.
  4. The wound is sutured using two layers of stitches or surgical clips to ensure clean healing with minimal scarring. Hair will eventually grow through these incisions, rendering the scar undetectable.

This skin removal method can lower the hairline by up to 5cm. In cases where the hairline has to be lowered by more than 6cm (a more invasive and complex procedure), a temporary tissue expander is used beneath the scalp.

A tissue expander is a balloon-like device, inflated with salt water that helps to stretch the skin and grow new cells prior to surgery. After 4-8 weeks, the skin of the scalp increases due to the skin growth and stretching, and the expander is removed.  In some cases, your surgeon will recommend that a small amount of hair grafting be performed at a later time to thicken the hairline and optimize results.

After Forehead Reduction surgery

Following your forehead reduction, patients will experience considerable swelling and bruising. Bandages will be used to keep the incision site clean and eliminate post-op swelling. Your surgeon will prescribe certain procedures to assist with any swelling or further complications in the healing process.

In 3-4 days patients may be able to resume driving and engage in light work and daily activities. Patients should keep their head elevated at a 45-degree angle to reduce swelling.

Forehead reduction surgery recovery

Forehead reduction surgery doesn’t usually require a significant recovery period. Most patients feel well enough to return to work after several days of rest. There may, however, be some swelling and bruising that lingers for a few weeks around the forehead, head or even eye area.

Moderate pain / discomfort is expected for 3 – 5 days. Patients can go back to their normal daily activities after maximum 1 week, but avoid strenuous exercise / activity for 6 weeks including any contact sports.

During your consultation, ask your surgeon to show you pictures of patients at various stages of post-operative healing so you can gain an idea of what you will look like and what to expect.

The first week following surgery:

. Following surgery, your head will be wrapped in bandages to minimize pain and swelling. Leave the bandages on until your surgeon agrees they can be removed. Dressings must remain on the incision site until the stitches are no longer necessary.

. Ice compresses will help to relieve any pain and swelling. Prescribed pain medication will also help if you experience prolonged discomfort. You should report any severe pain to your surgeon.

. Some surgeons may prescribe antibiotics to reduce the likelihood of infection occurring.

. Keep your head in an elevated position when you sleep for the first few days after surgery.

. It is normal to experience some numbness in the front of the scalp behind the incision. This numbness could last a few days or endure for several months while the nerves grow back.

. Itchiness may occur around the incision line as the nerve endings grow back. This is a good sign that the scalp is healing.

. The sutures will be removed after one week if they are not of the dissolvable variety.

. You can wash your hair two days after surgery.

Longer term outlook:

. Some hair loss at the front of the scalp may occur 2-4 weeks after surgery. Following a ‘resting period’ of several months, the hair will grow back in.

. Avoid strenuous exercise such as swimming, lifting heavy objects, going to the gym or running for at least two weeks, or until your surgeon advises you that you can resume such activities.

. Swelling may remain for several weeks or months after surgery. People heal at different rates, but rest assured, all swelling will gradually subside.

Hair Transplant method

Hair Transplant and Hairline lowering:

Hair transplant is not only considered as a reconstructive treatment but also used as a forehead reduction procedure making the forehead appears smaller by grafting hair to the forehead.

There are many people preferring to lower their hairlines, by doing hair transplant due to its advantages such as being minimally invasive in comparison with the surgical method and leaving no scars behind, however in the FUT method a long narrow scar may remain on the back of the head; but you should notice the facts that the scars from FUT method are less visible than the scars left behind by the surgical forehead reduction and nowadays, the FUT method is less common than the FUE method which leaves no scars behind at all.

After hair transplant procedure in Iran

After the hair transplant in Iran, your scalp may be very tender. You may need to take pain medications for several days. You should wear bandages over your scalp for at least a day or two. Your surgeon may prescribe an antibiotic or an anti-inflammatory drug for you to take for several days. Most people are able to return to work 2 to 5 days after the operation.

Within 2 or 3 weeks after hair transplant in Iran, the transplanted hair will fall out, but you should start to notice new growth within a few months. Most people will see 60% of new hair growth after 6 to 9 months. Some surgeons prescribe the hair-growing drug minoxidil (Rogaine) to improve hair growth after transplantation, but it’s not clear how well it works.

Recovery

Hair transplant procedure may take several hours to several days to complete. In part, this depends on the amount of work performed by the surgeon. You will go home the same day of the procedure.

Once the surgery is done, your surgeon carefully removes any bandages. The area may be swollen, so your surgeon might inject triamcinolone into the area to keep the swelling down.

You’ll likely feel pain or soreness at the transplant site as well as in the area where hair was taken from. For the next few days, your surgeon may prescribe:

Pain medications, such as ibuprofen (Advil)

Antibiotics to prevent infections

Anti-inflammatories, such as an oral steroid, to relieve swelling

Medications such as finasteride (Propecia) or minoxidil (Rogaine) to help stimulate hair growth.

Here are some aftercare tips for hair transplant surgery:

Wait a few days after the surgery to wash your hair. Only use mild shampoos for the first few weeks.

You should be able to return to work or normal activities in about 3 days.

Don’t press a brush or comb down over the new grafts for about 3 weeks.

Don’t wear any hats or pullover shirts and jackets until your doctor say it’s OK.

 Exercise for about a week.

Don’t worry if some hairs fall out. This is part of the process. Transplanted hair may not grow much or seamlessly match the hair around it for a few months.

How much does Forehead Reduction Without Surgery cost in Iran?

On average, you can expect to pay something between $500-2200 for forehead reduction surgery in Iran. However, there are different variables that will affect the final total cost:

. Where your plastic surgeon is located (those operating in larger cosmopolitan areas tend to charge more because of higher overhead costs).

. The reputation/experience of your surgeon

. The complexity of your particular case (more complex surgical cases, such as those where the forehead needs to be lowered a significant distance, or where the skin is very taut, take a longer time to complete).

. Whether or not a tissue expander (to stretch the skin) is required.

. The cost of anesthesia, nurses and operating facility fees.

. The type of method used (Surgical/Non-Surgical)

What is the Difference Between Hairline Lowering Surgery and Hair Transplant?

With forehead reduction surgery (hairline lowering), an incision is made across the hairline, the scalp is lifted, the hairline is advanced, excess skin is removed, and the incision is closed. In hair transplantation, hair is taken from the back and sides of the head, divided into tiny individual follicles, and then one hair follicle at a time is transplanted to the forehead skin. In most cases, a second hair transplant is required to achieve sufficient density. Although the end result can be similar between the two techniques, hairline lowering surgery is a much faster change and usually has better density than hair transplantation, unless multiple hair transplant procedures are performed.

What is the Difference Between Forehead Reduction Surgery and Brow Lift?

In a forehead reduction surgery (Hairline Lowering surgery), the hairline is lowered. In a brow lift surgery, the eyebrows are raised. In both cases, the size of the forehead may be reduced, but this is not the case for every type of brow lift. A brow lift is customized for each individual based on their anatomy, and techniques usually vary due to the position of the patient’s hairline.

How Much Can I Lower my Hairline? How Big is a “Normal” Forehead?

In most cases, doctors can lower the hairline by 2.0 to 2.5 cm in one surgical session. Keep in mind that human tissues stretch under tension, so removing 2.5 cm of forehead skin may result in lowering the hairline nearly 2.25 cm or more. This is different for each patient. It is also important to recognize that measurements are subjective. When placing a ruler against the forehead to measure the size of the forehead, the position of the rule can be slightly adjusted in different directions to drastically change the measurement results. Finally, thin, sparse hairs (“baby hairs”) along the frontal hairline often cannot be used to hide the scar, so the incision must be made higher into the hairline during surgery in order to use mature, healthy hairs for hiding the scar. This may cause the results of the surgery to appear slightly less in the postoperative measurement than the amount of skin removed during the surgery. This is all normal and part of forehead reduction surgery. Patients should not focus on the numbers for all of these reasons and should instead aim for improvement, which is almost always significant. There is no real “normal” forehead height. Doctors usually measure from between the eyebrows to where the thickest part of the hairline begins (not baby hair). The goal after surgery for most patients is to be in the 5.5 to 6.5 cm range, but this can vary. Many women look very beautiful with hairlines above this range. Hairlines less than approximately 5.5 cm from the eyebrows run the risk of looking very unnatural, odd, and aesthetically unappealing. For this reason, our doctors do not perform hairline lowering surgeries to lower the hairline beyond the 5.5 – 6.0 cm range.

Is Forehead Reduction Surgery Painful?

Forehead reduction surgery is a relatively painless procedure. Many patients will experience mild discomfort the evening after surgery, but it is usually not severe enough to need pain medication. However, in rare cases, there may be more severe pain and the doctor may prescribe medication to reduce it.

Is There an Alternative to Forehead Reduction Surgery?

There may be other options if you are not a good candidate for forehead reduction surgery: . Brow lift An alternative to forehead reduction surgery might be a brow lift if your forehead appears longer because of low brows. The procedure involves manipulating the muscles or moving the skin of the eyebrow area to lift the eyebrows on the face. In some cases, a brow lift can make the forehead appear shorter. . Hair Transplant Another alternative could be a hair transplant if your forehead appears longer because of a high hairline. This procedure involves removing hair from the back of the head and transplanting follicles along the front of the hairline. This procedure can also help shorten the forehead.

Does Forehead Reduction Surgery (Hairline Lowering) Look Natural?

If the incision is planned and the procedure is performed correctly, forehead reduction surgery usually results in a natural hairline. Thick hair hides the incision better, but generally, the incision is barely visible even in thinner hair.

Can a Patient Have More Than One Forehead Reduction Surgery?

In most cases, yes, a patient can have more than one forehead reduction surgery (hairline lowering), but it depends on the patient and their scalp laxity. The scalp generally stretches over time and this may allow another centimeter of forehead reduction if a second forehead reduction surgery is performed at a later date. In many cases, patients achieve sufficient improvement with one surgery.

The side effects of forehead reduction include: bleeding, pain, swelling, bruising, etc.
You can go back to their normal daily activities after 1 to 2 weeks.
Just like all the other cosmetic surgeries, the final results become apparent after a few months. This time period depends on the quality of your skin. There have been also cases who recovered a lot after a week.
