What is In Vitro Fertilization or IVF?

IVF stands for In-Vitro Fertilization which is sometimes called test-tube conception is the most effective assisted reproductive technology (ART) and a complex procedure in which mature eggs (ova) are retrieved from a woman’s ovaries and fertilized by male sperms outside of the body in a test tube before being transferred to her uterus, with the purpose of establishing a successful pregnancy. This procedures sometimes involves using donor eggs, sperms, or embryo in couples who can’t produce healthy sperms and eggs on their own.

The process involves stimulating the ovary to grow follicles (fluid-filled sacs in the ovary that contain eggs), monitoring this growth, when mature collecting these eggs via a small procedure, allowing sperm to fertilize the eggs in a laboratory, growing the fertilized egg in the lab and transferring it back to the womb after 3 to 5 days.

Steps involved in the IVF process

. Ovary stimulation

You should take gonadotropin — a fertility medication that stimulates ovaries to produce multiple mature eggs — 8-14 days before the beginning of your menstrual cycle. You should also take synthetic hormones such as leuprolide or cetrorelix that prevents too early egg release.

. Follicle development

You set a time to visit your doctor every two or three days while taking these medications so as to have your blood hormone levels checked and get ultrasound measurements of your ovaries done. This is for the purpose of monitoring the development of the ovarian follicles, where eggs mature.

. Trigger shot

You will get a ‘trigger shot’ when your follicles are ready. The injection helps the eggs to mature fully and ready to be fertilized. Some 36 hours after the trigger shot, you should visit the clinic to get your eggs retrieved.

. Egg retrieval

After receiving an anesthetic, your doctor inserts an ultrasound probe to check your ovaries and identify the follicles. And then, a thin needle is inserted to extract eggs, which are usually 8-15. Some women experience cramping or spotting for a few days after the egg retrieval process, but generally, you will feel better in a day or two.

. Fertilization

Now it’s time for your eggs to be combined with your partner’s sperm for fertilization. Before this, an embryologist, who is a specialist in sperm, eggs, and embryo, will examine your eggs.

If after a few cycles of IVF fertilization doesn’t happen or the sperm quality is low, the specialist may recommend an intracytoplasmic sperm injection (ICSI) procedure. In this method, sperm is directly injected into the ovum using a microinjection device.

If the eggs aren’t normal and cannot be fertilized, you should consider egg donation, which involves using donor eggs from a third party (an anonymous or known woman) to produce embryos that are to be implanted into your uterus for pregnancy.

. Developing embryo

After 3 days, some of the eggs that are successfully fertilized become 6- to 10-celled embryos and by the 5th day some of them become blastocysts containing fluid-filled cavity and tissues that will separate into baby and placenta.

. Embryo selection

Now the most feasible embryo(s) are selected by the embryologist and ready to be implanted into the uterus. If there are any extra embryo, they can be frozen to be used in future IVF cycles.

. Planting the embryos

Using a thin tube called catheter, the doctor inserts one to five embryos into the uterus through the cervix.

. Pregnancy

If you are lucky enough, the implanted embryo will grow into a baby. If you get more than one embryo, the chance of your pregnancy will be higher, and thus is the chance of having multiple pregnancy. That is why some 20% of babies born through in-vitro fertilization are twins, triplets, or even more.

Two weeks after embryo transfer, you can take a pregnancy test and it is worth explaining that in some women who are incapable of producing mature eggs on their own, immature eggs are retrieved and matured artificially outside the body through another process called in-vitro maturation (IVM).

How successful is IVF in Yazd Iran?

Beneficial factors in IVF success in Yazd?

There are many factors involved in success ratio rate of IVF in Yazd Iran including the age of the woman, causes of infertility, years of infertility, type of embryo transfer, quality of the sperm and whether self-eggs or donor eggs were used.

The success rate is higher when frozen embryos are used as opposed to fresh embryos. Success is usually higher in younger women, so even when a woman has an embryo transfer, say in her 40s with embryos that were created when she was in her 30s, her chance of success is that of a woman in her 30s. Frozen cycles tend to have a higher success rate as the embryos that survive the thawing process will be more robust, resulting in higher chances of achieving a pregnancy. Donor eggs likewise come from a younger donor who is medically fit and hence tends to give better success rates. The better the sperm quality the better the success rate.
