Success Rate
The ICSI procedure fertilizes 50 to 80 percent of eggs. You might assume all eggs get fertilized with ICSI-IVF, but they don’t. Fertilization isn’t guaranteed even when a sperm is injected into the egg.
Remember that fertilization rates don’t tell you the clinical pregnancy or live birth rates. Once fertilization happens, the success rate for a couple using ICSI with IVF is the same as a couple using regular IVF treatment.
If a woman gets pregnant naturally, there is a 1.5% to 3% chance that the baby will have a major birth defect. The chance of birth defects associated with ICSI is similar to IVF, but slightly higher than in natural conception.
The slightly higher risk of birth defects may actually be due to the infertility and not the treatments used to overcome the infertility.
Certain conditions have been associated with the use of ICSI, such as Beckwith-Wiedemann syndrome, Angelman syndrome, hypospadias, or sex chromosome abnormalities. They are thought to occur in far less than 1% of children conceived using this technique.
Some of the problems that cause infertility may be genetic. For example, male children conceived with the use of ICSI may have the same infertility issues as their fathers.
Will ICSI work?
ICSI fertilizes 50% to 80% of eggs. But the following problems may occur during or after the ICSI process:
. Some or all of the eggs may be damaged.
. The egg might not grow into an embryo even after it is injected with sperm.
. The embryo may stop growing.
Once fertilization takes place, a couple’s chance of giving birth to a single baby, twins, or triplets is the same if they have IVF with or without ICSI.

What is the average fertilization rate with ICSI?
The ICSI procedure fertilizes 50 to 80 percent of eggs. You might assume all eggs get fertilized with ICSI-IVF, but they don’t. Fertilization isn’t guaranteed even when a sperm is injected into the egg
New Scientist reports that a recent study revealed that ICSI boasts a pregnancy success rate of 24%. While IVF without ICSI still has a slightly higher pregnancy success rate at 27%, those aren’t bad odds for anyone who’s already tried other methods of assisted reproductive technology
Fertilization rates for ICSI: Most IVF programs see that about 70-85% of eggs injected using ICSI become fertilized. We call this the fertilization rate, which is different from the pregnancy success rate.
Pregnancy success rates for in vitro fertilization procedures with ICSI have been shown in some studies to be higher than for IVF without ICSI. This is because in many of the cases needing ICSI the female is relatively young and fertile (good egg quantity and quality) as compared to some of the women having IVF for other reasons.
In other words, the average egg quantity and quality tends to be better in ICSI cases (male factor cases) because it is less likely that there is a problem with the eggs – as compared to cases with unexplained infertility. Some unexplained cases have reduced egg quantity and/or quality – which lowers the chances for a successful IVF outcome.
IVF with ICSI success rates vary according to the specifics of the individual case, the ICSI technique used, the skill of the individual performing the procedure, the overall quality of the laboratory, the quality of the eggs, and the embryo transfer skills of the infertility specialist physician.
Sometimes IVF with ICSI is done for “egg factor” cases – low ovarian reserve situations. This is when there is either a low number, or low “quality”of eggs (or both). In such cases, ICSI fertilization and pregnancy success rates tend to be lower.
This is because the main determinant of IVF success is the quality of the embryos.
The quality of the eggs is a crucial factor determining quality and viability of embryos.
In some cases, assisted hatching is done on the embryos prior to transfer, in order to maximize chances for pregnancy.
