What is a Beard Transplant?

Beard Transplant is an implantation of hair follicles into any bald patches on the face. The hair follicles are removed individually from the back of the scalp as this is the area where the hair is the thickest, then the surgeon places each follicle in the indicated facial area that requires a thicker hair density. This gives instant fullness to the mustache or beard as required. If you are familiar with ordinary hair transplants, then the beard transplant applies the same technique.

Before Beard Transplant

Why can’t some Men grow beards?

. Genetics

Facial hair is produced by a chemical named dihydrotestosterone that is synthesized from testosterone (the hormone which encourages development and sexual characteristics in men). The ability to grow facial hair is given by the way in which the body reacts to testosterone.

Generally, men present the same levels of testosterone. However, men who can grow thick facial hair are more sensitive, or receptive, to testosterone than men who can’t grow beards. And this is where genetics come into play, the way a man’s body responds to testosterone results in how his beard grows.

. Skin conditions

In certain cases, however not often, a man might suffer from a skin condition which can limit beard growth. Alopecia is one of these conditions, causing hair loss and balding. If a man suffers from alopecia, his immune system attacks his hair follicles, accidentally, and this is where the hair growth stops. Luckily, the outcomes of alopecia are not permanent.

. Underactive thyroid

Another reason why some men can’t grow beards might be an underactive thyroid, also named hypothyroidism.

. Deficiency of iron or anemia

Another reason might be a deficiency of iron or anemia.

Pre-Operative Instructions:

. Ten days prior to your procedure: do not take aspirin, vitamin E, gingko biloba, or any multivitamins (as these contain vitamin E). Many medications also contain aspirin, so please be sure to read all medication labels. You may take an acetaminophen (Tylenol).

. Three days prior to your procedure: do not take any anti-inflammatory medications (e.g. ibuprofen), do not drink any alcohol, and do not over-expose to the sun.

. Make arrangements for transportation from the office after your procedure if you will be receiving any sedation during the procedure. The type of sedation you receive will be decided upon by you and the doctor. Most patients choose oral sedation, but we also offer twilight sedation which must be arranged ahead of time and requires you to have someone stay with you that first evening. This procedure typically takes between four and eight hours to perform.

. If you are traveling from out of town, please make the necessary arrangements for your hotel, flights, and transportation. Our office is here to assist you with your accommodations and we have a list of recommended hotels near the office.

. To help cover the donor site sutures, the hair in the back of your head needs to be ½ inch (2 cm) in length. If your procedure is being done by the FUE technique- which is the more common technique- the back and/or sides of your head will be shaved depending upon the number of grafts being transplanted. This may involve trimming a small area that can easily be covered by surrounding hair, or require more extensive shaving. You should plan accordingly and discuss this with your doctor and/or his assistants.

. For best healing, please refrain from smoking for one week prior to and one week after your procedure.

. Taking vitamin C, 1000 to 2000 mg a day for one week prior to your procedure may help with healing. To reduce the incidence of bleeding during surgery, vitamin K 50 to 80 micrograms a day, can be taken starting five days before surgery and continuing for two days after.

Day of your procedure:

. Have a light breakfast, but no coffee or caffeinated beverages in the morning as these substances will increase bleeding and sensitivity to medications. You will be provided lunch in the office and offered breaks during your procedure.

. Wear clothes that do not need to be pulled over your head to prevent dislodging grafts immediately after your procedure. Please remove all jewelry.

Who are the Ideal Candidates for a beard transplant procedure?

There are several categories of patients who cannot undergo beard transplant. These are usually patients who would not obtain good or any results after such transplantation, such as people suffering from certain conditions, like alopecia.

The number of patients who can obtain great results, on the other hand, is quite large and involves the following categories of men:

. Men who have suffered from various types of injuries and are left with scars on the face.

. Men who have previously undergone other surgeries and are left without one or more patches of facial hair.

. Men who were victims of burns can be very successful beard and moustache hair transplants;

. Men who have naturally less facial hair are usually the most common patients for these types of transplants.

What are the Advantages and Disadvantages of Beard Transplant?

Pros:

. Permanent results

. Safe surgical procedure

. Requires only local anesthetic

. Painless: perceived only as small ‘pinches’

. Transplanted hairs function as normal hair

. The new beard can be trimmed and shaved as usual

. Covers burns or scars permanently, if required

. Allows for various (and changeable) styles of beard.

Cons:

. Removal of hair from donor area

. Implanted hair will initially fall away

. The ‘revival’ period may take several months for new hair to grow

. Possible mild discomfort (bruising, swelling, increased sensitivity, redness, crusts and ingrown hair).

. Possible requirement of antibiotics or ointments to prevent infections

. No 100% guaranteed success. Sources show a 60-80% success rate and a second transplant might be required for full effect.

During Beard Transplant

Types of Beard Transplants

How do Beard Transplants work?

There are several techniques that can be applied for the procedure of beard transplantation. The difference lies in the method to be used for the extraction of the follicles and their implantation to the new growth site. The most common are the FUE, FUT and DHI methods.

. FUE: Follicular Unit Extraction

This is the most commonly applied procedure for beards. The surgeon will remove individual hair follicles from a donor region usually the scalp (The follicles situated on the back of the head) and implant them where you need your beard to be filled in. He will then make some very small incisions in the correct facial area. This is a very skilled and delicate process as it determines the direction, density and angulation of each new hair, and is entirely dependent on the expertise and experience of your chosen doctor.

Usually, it is estimated that between 4,000 and 7,000 hairs are needed for beard restoration, depending on how large or patchy the recipient area is, and this surgical technique lasts from four to six hours under local anesthetic. Tiny scars will occur where the hair follicles were removed, but these are not noticeable as the normal hair will cover them adequately. Transplants of this kind can be made for sideburns, a goatee or moustache.

. FUT: Follicular Unit Transplantation

FUT also known as the strip procedure, the surgeon performing the FUT method removes a strip of healthy hair-bearing skin from the donor area (again, most typically the scalp), and implants it into the facial areas needing to be treated. The donor area will be small and stitched and is soon covered by the surrounding hair making it unnoticeable.

As with FUE, tiny incisions are made in the beard and the collected hair is applied. The hairs are not collected individually but rather in groups of one to four 4 hairs, called follicular units. This is one of the reasons why the beard looks fuller using this method as generally there are more hairs transplanted close together. This surgical process might last between four and eight hours, depending on the area to be covered and the number of units to be relocated; and again the procedure is normally carried under local anesthetic.

. DHI Beard Transplantation

DHI (Direct Hair Implantation) is one of the most effective methods used in the treatment of thinning / balding beard or moustache. Unlike the FUE method, in the DHI beard transplant method, the preparation of the recipient site and implantation of the follicle are performed simultaneously, that is, the extraction and implantation are combined as part of a one or two step process. The implantation of beard hair follicles extracted directly from the recipient areas is done without the need to create channels in the recipient area, using an implanting pen (Choi pen).

The average indicator for different styles or facial areas is:

. 500 – 800 grafts per mustache

. 600 – 1200 grafts per full goatee

. 200 – 350 grafts per each sideburn

. 1000- 2000 grafts per cheek beard

After Beard Transplant

Post-Operative Instructions:

. Medications: These are the medications that will be given to you and/or prescribed by the office. If you are not able to take any of them (i.e. due to allergies) please let us know before your procedure. This will allow us to properly call in your prescription to your local pharmacy.

. Keflex – antibiotic to prevent infection. Take one pill twice a day for three days.

. Tylenol #3 – mild pain reducing pill containing hydrocodone. Take one to two pills every four to six hours as needed for any discomfort in the donor area. Advil (Ibuprofen) and Aleve (Naproxen) are also very effective and should be taken with the earliest sign of discomfort. They can be taken with the Tylenol #3 or Percocet for best results.

. You will be given prescriptions for Percocet (a stronger pain pill) and Ambien (for sleep) on the day of your procedure. These prescriptions can be filled while you are having your procedure.

. Antibiotic ointment – provided to you for you to apply to the donor area twice a day for five days after the procedure.

Sleeping and Activity:

. For the first night, sleep with your head elevated on several pillows or in a recliner chair to prevent swelling, and place a towel under your head for possible light bleeding.

. Avoid any strenuous physical activity the first two days after your procedure to prevent increased swelling and loss or damage of grafts. After six days, you may resume moderate exercise, weight lifting, etc. After 10 days, you may resume swimming.

. No direct sun exposure to the face for more than 30 minutes at a time for the first two weeks. Wearing a hat provides adequate protection.

Caring for the transplants:

. There will be tiny crusts where each graft was placed that will fall off at four to six days. Gently rub off any remaining crusts at one week. If there is any discomfort or bleeding, stop rubbing and begin to rub again two days later. There will also likely be a mild amount of pinkness to the area for a few days after the crusts fall off.

. You can begin to shave at eight days after the procedure.

Most of the transplanted hairs will fall out typically by two to three weeks. This is normal, as the hairs will start to grow back four months later. Once the hairs start to grow back, you can shave them normally.

. Most patients are presentable at five to seven days after without any obvious signs of having had a procedure other than they will have new hairs and possibly some mild flaking. There is a chance you will have prolonged pinkness. We recommend you take Benadryl 25 mg twice a day to treat this.

. For the first five days after the procedure do not get the transplanted areas wet. On the sixth day, you can begin to wash your face normally, and full exercise is permitted. Normal hair shampooing is permitted the day after your procedure.

. If your procedure is done by the FUE technique, no sutures are used. If your procedure is done by strip, you can receive absorbable sutures and thus do not need to have them removed as they will dissolve. If non-absorbable sutures are used, they can be removed at 10 to 12 days by a family member, friend, or local doctor using the kit provided by the office. If you live close to one of our offices, you can return for suture removal at 10 to 12 days.

What to expect:

. Numbness, tingling, or similar sensations at the graft sites and along and above the donor area in the scalp is common and normal. This will generally take several weeks, occasionally several months, to resolve on its own.

. You may have your hair cut at one week. Hair coloring is permitted two weeks after your procedure.

. Once transplanted, the hairs will grow just like normal beard/mustache hairs. They can be shaved or allowed to grow out.
